Case Manager
Location: Bluefield, West Virginia
Description: Diamond Healthcare Corporation is at present looking to employ Case Manager right now, this job will be assigned in West Virginia. The Case Manager provides intake/assessments, psychosocial assessments, quality assurance, group activities, case management, connection to community resources, utilization revi! ew and crisis intervention as part of the multi-disciplinary treatment team.
REPORTS TO: Director of Outpatient Services, Medical Director of Psychiatry, Regional Director or designee.
8:00-4:30 Monday through Thursday AND 11:30 until 3:30 on Friday.
Bachelor’s Degree in Social Work, Psychology or related field required.
1+ year(s) of experience in working with mental health or chemical dependency field.
Good communication and organization skills.
Training in non-violent crisis intervention.
Current CPR certification.
SPECIAL KNOWLEDGE AND SKILLS: Knowledge of or experience in mental health or chemical dependency fields; ability to work under pressure and meet deadlines; ability to adapt to workload demands and set priorities; positive, professional presentation; ability to build rapport quickly and develop working relationships; ability to work cooperatively with the program staff; good comm! unication and organizational skills. Clinical experience suffi! cient to perform required services. Requires access to transportation.
