WES Health System has an exciting opportunity available f! or a full-time Certified Recovery Facilitator in Our Mission Is Yours Program.
FUNCTION : To coordinate and provide counseling and rehabilitative services for the participants of the Recovery Path Program.
RESPONSIBILITIES :
Carry a caseload of OMY participants.
Assist in the assessment and evaluation of participants to establish recovery plan goals, objectives, and appropriate activities.
Provide appropriate education/training in workshops/groups, skill building/vocational training, i.e., individual, group, and family supports; community activities, socialization, milieu therapy, and psychosocial treatment for the OMY participants.
Create and implement recovery focused curriculum as it applies to group and workshop topics.
Assist the participants, when necessary to accomplish their activities of daily living.
Monitor all aspect of the participantâs recovery plan on your caseload; including community integration, emp! loyment potential.
Collaborate with Psychiatrist and thera! pist for medication monitoring, crisis planning/prevention. Collaborate with case manager for residential/housing/benefits/resources, and other service providers in coordination of recovery planning for assigned caseload of participants.
Provide individual recovery focused supports to individuals when necessary.
Travel with participants when needed via public transportation and or vehicle.
Provide educational/training supports within community and or site based locations
Provide crisis intervention when needed.
Back up other staff when necessary.
Attend and participate in staff meetings.
Complete at least 19 hours per year of in-service education related to job responsibilities.
Perform related duties as directed.
