The LPN assesses,monitors, and ensures access to coordinated medical and mental health services as part of
an Assertive Community Treatment team.
- Administer intramuscular injections and/or supply medications to consumers as prescribed by psychiatrist.
- Meet with all residents in the community on an ongoing basis to monitor health and to monitor and assess consumer progression/regression in behavior and/or appearance. Meet with residents with unst! able medical or mental health status more often.
- Work! with consumers in filling prescriptions and coordinating appointments as needed.
- Refer residents to emergency care if needed, work with emergency responders/emergency room staff to coordinate emergency treatment, coordinate treatment and discharge planning with hospital staff.
- Maintain CPR/First Aid Certification.
- Maintain up to date, accurate, complete, and timely consumer chart documentation.
- Attend staff meetings, case conferences, group supervision, and trainings as needed.
- Comply with all SUS Employee Policies, Procedures, and Protocols, SUS Corporate Compliance Policies, and NYS Dept. of Social Services Rules, Regulations, and Statutes.
- Assist full time Registered Nurse.
- Record keeping as per agency regulations.
- Associate’s degree in Nursing; License in NY State.
- Two years of experience in direct clinical work with psychiatrically disabled populations st! rongly preferred.
- Ability to work independently and as part of a team, to assist adults with severe mental illness of diverse ethnicities, backgrounds, ages, and sexual preferences.
