Job Summary: Under the dir! ection of the IBH Program Manager, will provide behavioral health counseling, psychotherapy, treatment services, and limited case management services. Participate in a multidisciplinary approach with the psychiatrists and primary care physicians. Part-time position.
Specific Functions
Patient Care Responsibilities:
Obtains pertinent patient data from patients, family, significant other, other health care providers and medical record as necessary; Participates (as assigned) in patient care conferences.
Provides assessment, recognition, and treatment of mental disorders and psychosocial problems; Assist, in clientâs community functioning by linking patients with public benefits, vocational rehabilitation, social support, and other community services; Provides community referrals for more complex psychiatric needs; Assists psychiatrist and/or primary care doctor in treating and managing mental health and psychosocial issues; Provides brief focused! intervention for clients in need of mental health services, including individual and group interventions; Provides ongoing psychotherapy and support to clients with chronic emotional and mental health conditions; Provides depression care management for patients with co-morbidity depression and diabetes including the following tasks; Review of PHQ-9 patient health questionnaires; Intake with identified patients; Use of Behavioral Activation techniques or optional evidence-based brief structured psychotherapy; Coordinates and facilitates communication between patient, primary care physician, and consulting psychiatrist; Supports clinic when child / elder / domestic abuse mandatory reporting is required; Initiates Mandated Tarasoff Reporting when required; Participates in emergency procedures; Performs failed appointment actions as ordered by the M.D./Psychiatrist
Education:
Assists in maintaining adequate levels of appropriate patient education materials;! Identifies needed educational literature that is unavailable to the Ma! nager; Directs patients / significant others / family requesting educational classes to the outside resources as needed; Documents any teaching done in the patientâs medical record; Acts as a resource for other staff; Participates in Continuing Education programs that contain content on behavioral health, clinical issues, quality improvement, moral, ethical and legal issues; Maintains working knowledge of the Ambulatory and Community Health Services policies and procedures; Participates as a preceptor to new employees as assigned by the Manager.
Quality Improvement / Standards:
Complies with current applicable standards; Participates on task forces / quality teams / clinical practice groups as needed and/or requested by the Manager; Maintains competency for polices / procedures related to Behavioral Health Clinics.
Skills
Qualifications:
Masterâs Degree in Social Work from an accredited college or university which qualifi! es licensure as a Licensed Clinical Social Worker
Must possess a valid California license as a Licensed Clinical Social Worker (LCSW)
Maintains HIPAA compliance
Excellent written and oral skills, ability to organize and present information
Proficiency in Microsoft Office software.
